Wanting a bigger penis can be a positive, personal choice, no different than a woman who wants bigger breasts. Some men have always wanted more girth, length, flaccid presence, or confidence. Others remember being longer, thicker, fuller, or less retracted than they are now. Penile size can change because of blood flow, erectile function, tissue health, Peyronie’s disease, prostate treatment, hormones, weight, aging, or previous procedures.

Blood flow, erectile dysfunction, and lost size

Strong erections repeatedly fill penile tissue with blood and oxygen. When circulation declines and erections become softer or incomplete, the penis may look thinner or shorter and may no longer expand to its previous size. Over time, inadequate oxygenation can contribute to smooth-muscle loss, fibrosis, reduced elasticity, and actual loss of penile tissue. [4]

Viagra® (sildenafil), Cialis® (tadalafil), Levitra® (vardenafil), Stendra® (avanafil), TriMix, BiMix, and QuadMix may create a temporary medication-assisted response in some men, but they do not remove arterial plaque or rebuild lost penile tissue. Peyronie’s disease

Peyronie’s disease can cause much more than curvature. Plaque can prevent part of the penis from expanding normally, leading to shortening, narrowing, indentation, hourglass changes, lost girth, instability, and weaker erections. In one study, 85% of men reported lost length before their first evaluation, averaging about 1 inch; volume-loss deformities are also common. [3,5]

Prostate surgery and prostate cancer treatment

Penile shortening after radical prostatectomy is well documented. [6] Nerve injury, reduced blood flow, fewer erections, hormonal treatment, fibrosis, and tissue changes can affect length, girth, fullness, sensation, and erectile quality after prostate cancer treatment.

Aging, hormones, and weight

With age, changes in collagen, elastin, circulation, hormones, metabolism, and erection quality can reduce fullness and expansion. Low testosterone can contribute to weaker erections and incomplete expansion, while weight gain can conceal part of the shaft beneath the pubic fat pad and make the penis look shorter.

Trauma, scarring, and previous enlargement procedures

Trauma, surgery, permanent fillers, silicone, fat transfer, self-injected substances, and poorly performed penile enhancement can cause scarring, asymmetry, filler migration, narrowing, curvature, sensitivity changes, or an unnatural appearance. What is the average penis size?

A systematic review of clinician-measured data from up to 15,521 men found an average flaccid length of about 3.6 inches (9.1 cm), an average erect length of about 5.2 inches (13.2 cm), and an average erect circumference of about 4.6 inches (11.7 cm). [8] Average is a statistical description—not an ideal size.

What penis size do women prefer?

In a study using life-sized 3D models, women selected larger than the statistical average. Preferred long-term-partner size averaged about 6.3 inches in length and 4.8 inches in circumference; for a casual partner, about 6.4 inches and 5.0 inches. [9]

Does girth matter more than length?

For many women, girth can create more stretch, pressure, friction, and contact than additional length. Research has found that women may place greater importance on girth than length for sexual satisfaction. [10,11] Greater girth can also increase contact near the vaginal opening and anterior (front) vaginal wall, including tissue associated with the internal clitoral complex. [12,13]

When both partners have changed

Sometimes penile girth, firmness, and expansion decline at the same time a female partner develops vaginal laxity after childbirth, perimenopause, menopause, postmenopause, or aging.
